Our team approach to care means that you don’t have to jump from doctor’s office to doctor’s office looking for answers to manage your diabetes. We work with your primary care and specialty care doctors to get you the diabetes care and treatment you require.

What Are the Different Types of Diabetes?

Type 1

Your body’s immune system attacks and destroys the cells in your pancreas that make insulin. This is usually diagnosed in children and young adults, although it can appear at any age.

Type 2

Your body either does not make enough insulin or is not able to use it properly. This is the most common form of diabetes and is typically found in middle-aged and older adults.

Gestational

This form of diabetes is found in pregnant women. Usually, once the baby is born, the diabetes disappears. However, if you’ve had gestational diabetes, you are more prone to developing type 2 diabetes later in life.

The Sweet Life, Diabetes & You Program

Have you been diagnosed with diabetes? The Sweet Life is a six-month diabetes self-management education program designed for people diagnosed with diabetes. Our focus is patient-centered and individualized to meet your treatment goals so that you can become more confident in the management of your diabetes. We provide education and ongoing support that will aid in medication management, provide blood sugar guidance and promote healthy lifestyle goals through exercise and nutrition. Check with your Baptist Health Primary Care physician to ensure a program like ours will benefit you.

This causes your blood glucose, or sugar levels, to become too high, which can cause health problems such as heart disease, stroke, kidney disease, dental disease and others.

  • Our comprehensive diabetes management and education program, Sweet Life, is accredited and recognized by the American Diabetes Association for Diabetes Self-Management Education (DSME) and for Diabetes Self-Management Support (DSMS).

    After an appointment with our board-certified physician, you meet one on one with a dietitian and attend a comprehensive three-hour education class on managing your diabetes.

    The program includes:

    • Review of lifestyle goals
    • Testing for A1C, cholesterol and blood pressure levels
    • Meal planning and nutrition counseling
    • Exercise planning
    • Medication management
    • Lifestyle modifications
    • Prevention of complications

  • Currently, you must be a primary care or endocrinology patient of Baptist Health to enroll in our program. It’s always best to check first with your primary care physician to ensure a program like ours will benefit you and with your insurance company to see if our services will be covered.
