Verify Credentials

Baptist Health South Florida Credentialing Verification Office provides credentialing services for all Baptist Health South Florida (BHSF) facilities and delegated credentialing for the Baptist Health Medical Group (BHMG). The CVO department is responsible for both the Initial and Re-credentialing processes for all credentialed practitioners. Other services offered by the CVO department include, but are not limited to, system-wide tracking of licensure, DEA certificate, board certifications, medical malpractice, and biographical information.

The Provider Services Department provides credentialing services for all Physician Office Managers, Staff and Scribes who require access to the Baptist Health systems, systemwide. When requiring assistance with office staff access or office staff updates, please contact the Provider Services Department at their main line for assistance at 786-595-9723 or via email at ProviderServices@BaptistHealth.net.
